Why do I have varicose veins?
Varicose veins are caused when the valves in the veins are not working properly. It is those valves that push the blood back to your heart. If they are not doing their job properly then that causes the blood to stay in your legs, which can cause a variety of symptoms, some that are merely cosmetic and some that are painful and dangerous to leave untreated.
The best way to assess these veins is with ultrasound. Ultrasound will show us exactly what is happening in the veins and determine if they are working properly. If indeed the ultrasound indicates a problem, then we will explore treatment options, such as ClariVein or laser to shut off the vein that is not working properly.
Once the main source of the problem is gone, you should see great improvement of the veins in your legs. Sometimes multiple procedures need to be done because there is more than one vein that is not working correctly. Once we have taken care of the main problematic veins, we can work on spider veins. Some people may have only spider veins which can be shut down with sclerotherapy.

For superficial vein problems can be treated with a short, pain free procedure which closes the problem vein in a single session. ClariVein works by inserting a very thin and flexible catheter into the vein (not much bigger than a thread.) The tip of the catheter spins to irritate the interior wall of the vein – no pain! At the same time a sclerosing solution is sprayed through the catheter. This double process insures that the treated vein will collapse and no longer hold blood. EVA – Endovenous Ablation
Treating superficial leg veins with Endovenous Ablation or EVA, is a minimally invasive alternative to surgical vein stripping. An ultra-thin optic fiber is passed into the vein interior. Specific laser light or radio frequency energy is pulsed into the vein as the tiny fiber tube is withdrawn and the vein begins to collapse and seal shut. This action relieves much of the pressure that “feeds” smaller spider veins and improves the appearance of the treated vein itself. After treatment you will wear compression hose. Most patients return to daily routine in 1 or 2 days with some soreness for a week or two. Endovenous Ablation is FDA-approved for the treatment of the great saphenous and other leg veins. Most insurance plans now cover EVA for symptomatic varicose veins. No scars, no stiches and only a day or 2 of lost productive time. Only a local anesthetic is required.
Sclerotherapy is a minimally invasive procedure to treat uncomplicated spider veins and uncomplicated reticular veins. The treatment involves the injection of a solution into the affected veins. We use FDA-approved Asclera® because of its impressive comfort and effectiveness. Sclerotherapy is sometimes assisted with ultrasound in treating larger veins that are deeper in the skin. Spider veins are very small and very fine red or blue veins closer to the surface of the skin. They can look like a thin red line, tree branches or spider webs. Reticular veins can also be known as feeder veins. They are the blue and green veins beneath the surface of the skin. Reticular veins enlarge because of increased pressure in the vein. Sclerotherapy works by damaging the cells lining the inside of blood vessels. Over time this causes the vein to clot and eventually be replaced with naturally generated tissue.

Laser to treat spider veins is becoming more popular since it is a completely noninvasive treatment. The laser light passes through the skin and is absorbed by the “red” color of hemoglobin. The absorption causes the vein to heat up very quickly, resulting in coagulation of the blood fluid in the vein. Successive treatments are usually required to get the best clearance since there is still pressure from the deeper veins that attempts to re-expand the vein. After each treatment the pathway is narrowed and is eventually closed off. The vein is then absorbed by the body and the blood path is diverted to deeper veins. Laser treatment of veins is fast and not particularly painful. The sensation is similar to the snap of a rubber band.
When surface varicose veins are too large to treat with sclerotherapy and are too small to treat with endovenous ablation, microphlebectomy can treat them. This is a good solution for bulging varicose veins that cause pain, tenderness, skin weakening, itchiness, or are simply unsightly. With microphlebectomy, varicose veins are removed through small punctures, which leave virtually no scars. This is a comfortable procedure requiring only local anesthetic. The veins are accessed and removed through a series of pinhole perforations using a special set of instruments. This is NOT the same as vein stripping. Because microphlebectomy is performed through small needle punctures, it is far less invasive than vein stripping, which is performed through incisions.

Is laser leg vein treatment better than injection treatment?
Both treatment modalities are effective in treating leg veins. During your free consultation, you will be advised as to which treatment will be best for you. Some veins are best treated by one specific procedure and sometimes with a variety of procedures. Since vein structure, width, depth and location all play a part in the selected process, only a physician assessment can provide you with the correct options.
Is the treatment painful?
Sclerotherapy is quite comfortable and with the new Asclera products, patients find it is very well tolerated with no need for numbing. Other treatments may be more comfortable with certain anesthetic techniques.
Will it be necessary to wear compression stockings after treatment
Depending on the procedure, you may or may not need to wear compression stockings. If required, they usually are only necessary for a few days. With laser, typically you won’t need them
Will it require more than one treatment?
You should plan on several sessions for both laser and sclerotherapy. EVA and Microphlebectomy usually resolve the issue for the specific treated vein in a single session.
Will I see results immediately?
With some treatments, yes. Others make take more time. Again, your assessment physician will explain all the expectations associated with the recommended treatment.
